- Biometrics of the child and the parent (or guardian in the absence of parents).
The child’s photo is captured.
- Required documents will be submitted, including the original juvenile card, the child’s birth certificate (CRC), and CNICs of parents or guardians.
- The form needs to be attested by a blood relative or gazetted officer.
- Fees are paid based on the selected service (Normal/Urgent/Executive).
